PETITIONER:

Birth Certificate if born in the U.S. or Certificate of Naturalization / Permanent Residency Card

Social Security Card and Drivers license

Marriage Certificate (if applicable)

Birth Certificate

Divorce decree or death certificate of any previous spouses, along with their information: full name, date of birth, date and place marriage took place.

If petitioning spouse: Addresses where you have lived during the last five years, including month and year you started living there and date you moved out.

If petitioning spouse: Name and addresses of employers you have worked for during the last five years, including month and year you started to work there and the date you left.

If petitioning spouse: 2 passport-style photographs

BENEFICIARY:

Birth Certificate, current or expired Passport, and Personal Identification Document.

Marriage Certificate

Spouse’s birth certificate and your children’s birth certificates

Tourist Visa (if applicable)

Work authorization, Social Security, and I.D.  (IF APPLICABLE)

If spouse is petitioning: Addresses where you have lived during the last five years, including month and year you started living there and date you moved out.

If spouse is petitioning: Name and addresses of employers you have worked for during the last five years, including month and year you started to work there and the date you left.

Money order for $535.00 for USCIS (preferably from the Post Office).

If your spouse is petitioning you: 2 passport-style photographs

Office Fees: $1,000.00
